As a Community Health Care Assistant working in the out of hours district nursing service you will be integral to the care of the patients.

The Trust is focused on supporting the Community and helping patients remain independent in their own home. You will actively assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting, including supporting end of life patients.

The evening and overnight district nursing team supports patients from 19:00-08:00 across a 7 day week.

The core hours of this post are 19:00-00:00, 3 evenings per week, covering weekends and bank holidays.

You will work alongside a member of the team with full support of your Team Leader and other members of the team. Your caseload will be varied to your skills and training requirements. It will have a variety of patients with health and social care needs. This can vary from simple and complex patient care to looking after people at the end of their lives who wish to remain at home. You will work closely with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is appropriately coordinated.
