East Bergholt Surgery and Constable Country Medical Practice in Colchester is seeking a Senior Practice Nurse for a full-time position, available on a 12-month fixed-term contract. The role involves working at both practice locations and offers flexibility with a choice of 4 compressed days or 5 shorter days. Responsibilities include conducting health assessments, administering treatments and medications, performing diagnostic tests, and developing personalized care plans for patients with long-term conditions.

Additionally, the Senior Practice Nurse will manage and review long-term conditions in accordance with established policies, support women's health services, implement vaccination programs, and provide wound care. The position also encompasses managing chronic conditions such as asthma, COPD, and diabetes through dedicated clinics and reviews, all while ensuring adherence to evidence-based practices and maximizing patient health outcomes.
