Band 5: £29,970 to £36,483 a year  PERMANENT  GOOD SALARY 

An exciting opportunity has arisen for a Band 5 Mental Health Practitioner to join our friendly and dynamic team and to work specifically within our Memory Service.

We are looking for an enthusiastic and experienced Practitioner who has a keen interest and previous knowledge in Dementia and will play an integral part in delivering high quality care for our patients. Our Memory Assessment Service provides early identification, assessment, diagnosis and support to people with memory difficulties. The successful candidate will be able to work autonomously as well as within the team and have excellent communication and organisational skills. They will be motivated to learn and develop their skills within the Memory Assessment Service with the support and Supervision from highly skilled members of the team.

The Post holder will be part of the Community Mental Health Team and be expected to contribute to the wider working of the team, including participating in the Duty system and supporting with Students and Junior Staff.

The supportive leadership team promote the development of our services to provide the best outcomes for our patients and we know that to achieve this we need to invest in our staff through creating a good working environment and providing extensive training opportunities.

Ability to drive and use of a car for work purpose is essential in this role.

You will hold Clinics and complete an initial assessment, including cognitive testing, for routine patients referred for a Memory Assessment.

You will participate in Supervision sessions to discuss assessments and formulate care plans regarding diagnosis, treatment and Post Diagnostic support as required.

You will have excellent communication skills in order to gain valuable collateral information and deliver sensitive information to patients and family members.

You will have excellent organisational and IT skills in order to maintain the documentation required following assessments.

You will participate in the Duty system and triage referrals into the team as well any Urgent issues for patients on the active caseload.

You will provide support and guidance to junior members of staff and students.
