The role of Community Mental Health Practitioner (Memory Assessment Service) in Eastleigh with the Hampshire and Isle of Wight Healthcare NHS Foundation Trust involves joining a dedicated team focused on providing quality care to individuals with memory difficulties, with a specific emphasis on dementia. The successful candidate will conduct assessments, including cognitive testing, and participate in formulating care plans while collaborating closely with team members. Excellent communication, organisational, and IT skills are essential, as well as the ability to work autonomously and as part of the team.

Additionally, the Practitioner will engage in support and supervision of junior staff and students, participate in the Duty system for triaging referrals, and contribute to the wider community mental health team efforts.
