The Lead Nurse for Long-term Conditions at Haringey GP Federation in London is a full-time, permanent role focused on the management and delivery of services for patients with conditions such as Hypertension, Chronic Kidney Disease, and respiratory diseases (Asthma, COPD). Reporting to both the GP Clinical Lead and the Head of Integrated Services, the successful candidate will work within General Practices in East Haringey, community centers, and potentially conduct home visits. The position requires experience in caring for patients with long-term conditions, with an emphasis on managing a personal caseload, supervising junior staff, and familiarity with healthcare record systems such as EMIS Web or System One.

Ideal candidates will be a General Practice Nurse or a secondary care nurse eager to develop their clinical competencies, with the ability to work at an advanced level and adherence to national guidelines.
