ACCOUNTABLE TO: GP Clinical lead and Head of Integrated Services
HOURS: Full-time
SALARY: £44,000 to £52,000 for WTE (37.5 hours) subject to experience
CONTRACT TYPE: Permanent
MAIN LOCATION: Working with General Practices in East Haringey, community centres, Federation main office and potentially home visits.
OFFICE LOCATION: 48 Station Road, Wood Green, N22 7TY

We are looking for a Lead Nurse with experience or looking to develop clinical competencies in managing long-term conditions in general practice, specifically Hypertension (HTN), Chronic Kidney Disease (CKD) and respiratory disease (Asthma, COPD).
This exciting role will hold responsibility for the delivery of long-term conditions (LTC) service at Haringey GP Federation. This role is open to a general practice nurse, or a nurse from secondary care with This could be a great opportunity for a General Practice Nurse, or nurses in secondary care, with relevant experience in managing patients with long-term conditions who is interested in progressing their career and leadership skills.
Ideally the candidate will have:
Experience caring for patients with long-term conditions and managing own caseload of patients
Experience working in a team, supervising junior members of staff
Experience of EMIS Web or System One;
Be aware of national guidelines and management of specific patient groups
ARTP Spirometry Accreditation or be willing to work towards accreditation.
Experience working at an advanced level, caring for patients with long-term conditions;
Manage own caseload of patients.
