Job description
An opportunity has arisen for an experienced Advanced Nurse Practitioner (Digital Badge Holder) to work alongside our friendly, established team of staff based in Wymondham, Norfolk.
Pay: Dependent on Experience (ARRS Funded)
- Supporting the practice by being one of the duty clinicians alongside other ANPs and GPs.
- General health and wellbeing promotion as well as creating individual, person-centred plans for patients with, or at risk of, long term condition.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effecting emergency care.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- Meeting the needs of patients presenting for opportunistic wound care.
- Practicing autonomously as an Advanced Nurse Practitioner providing patient centred clinic care.
- Process and communication results of tests to patients and ensure that a care plan is in place.
- Maintain accurate, up to date patients records on the practice computer system (SystmOne).
About us
- To deliver a high standard of patient care as an Advanced Nurse Practitioner (ANP) in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
- Working in a flexible approach and in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.
- To manage own clinical workload in general practice, working within their own professional boundaries, responding effectively to patient need and ensuring patient choice and ease of access to services.
- To provide education and training of students and other colleagues.
- To proactively support future development of the practice and share the Practice values of Excellence, Responsibility, Integrity, Development and Commitment.
- The candidate will work according to practice protocols and guidelines, delivering care independently and in collaboration with GPs and the winder practice team.
- To be responsible for the daily delivery of practice nursing service on a dedicated site and working as part of the practice multidisciplinary team, delivering care within their scope of practice to the practice population.
- To work independently to provide and maintain a high professional standard of nursing care in adherence to all Practice protocols/policies & guidelines, as well as to provide nursing assistance, where required, to other members of the primary healthcare team.
- The role of ANP needs to be flexible in line with the present atmosphere for change within the health service delivery. It is essential that the nurse works within own level of competency and expand their role in accordance with practice requirements and own scope of practice.
