CASELOADMENTAL HEALTHNHSSOCIAL WORK
The Primary Care Mental Health Liaison Practitioner (MHLP) role with Sussex Partnership NHS Foundation Trust in Bognor Regis involves providing critical support within GP surgeries for patients experiencing moderate mental health issues. The successful candidate, who should be an experienced Band 6 Mental Health Practitioner (such as a Mental Health Nurse, Occupational Therapist, or Social Worker), will deliver 6-8 sessions of cognitive-behavioral therapy (CBT) informed interventions, perform initial assessments, and work collaboratively with GPs, local agencies, and IAPT services. This position aims to enhance the connection between primary and secondary care, ensuring smooth transitions and comprehensive care plans for clients.

Additionally, the role includes maintaining relationships with secondary care teams, developing care pathways, providing training to primary care staff on mental health topics, and ensuring access to local resources. The position offers an incentive of up to £8,000 for eligible practitioners, along with a relocation package under certain conditions, emphasizing the Trust's commitment to attracting qualified professionals to support mental health care in the community.
