Riverbank Medical Centre, Midhurst has an exciting opportunity for a hardworking and enthusiastic care coordinator to join our friendly practice.
To provide administrative support to the Doctors and Health Professionals at the surgery and a coordinated approach for patients
Purpose of job/responsibility:
Coordination, recall and booking for the following as required:
- Frailty / Advance Care Planning
- Learning Disability reviews
- QoF recalls
- Dementia reviews
- Severe Mental Illness reviews
- Post natal and baby checks
- Locally enhanced services monitoring
- PCN DES and IIF targets
- Liaising with patients, hospitals, social services and other outside agencies.
- Liaison between the clinical team, patient and hospital specialists in order to coordinate outpatient and communitycare
- Lead on nursing homes and be their first point of contact with the practice
Assistance with general administrative tasks including:
- Scanning and document processing
- Repeat prescription processing
- Appointment booking
- Vaccination schedule monitoring and planning
- Gathering statistical information as required
- Main reception duties

This Job Description is neither exhaustive nor exclusive and will be reviewed periodically in conjunction with the post holder. The post holder is required to carry out any duties that may reasonably be requested by the Practice Manager and Partners.

Using personal security systems within the workplace according to Practice guidelines
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
