The Newcastle upon Tyne Hospitals NHS Foundation Trust is seeking a dynamic and experienced Band 6 Nurse Specialist to temporarily join their community cardiac rehabilitation team during a maternity leave. The ideal candidate will possess cardiology experience, exemplary communication skills, and the ability to supervise junior staff. Responsibilities include leading cardiac clinics, conducting home visits, and delivering rehabilitation programs while managing a personal caseload of patients recovering from various cardiac conditions. A full UK driving license and access to a car are essential for this role.

The Nurse Specialist will work autonomously, assisting patients post-myocardial infarction, pre- and post-surgery, and those with heart failure, addressing their psychological, physical, and emotional needs to ensure a smooth transition from hospital to home. This role involves evaluating and delivering patient-centered care, educating patients and families, and collaborating with GPs for medication management. The position requires strong organisational and IT skills, along with a commitment to advancing nursing practice in cardiology. Flexible working arrangements may be accommodated.
