Our growing multi-disciplinary team, which includes Doctors, Pharmacists, Pharmacy Technicians, Nurses, and a team of Administrative and Reception staff is committed to delivering high-quality, patient-centred health care while embracing innovation and collaborative working.

We also undertake vocational training of new General Practitioners, Foundation Pharmacists, Practice Nurses and undergraduate Medical Students.

As part of our multi-disciplinary team (MDT), you will:

• Hold face-to-face and telephone medication reviews, optimising treatment for long-term conditions

• Provide expert medicines advice to GPs and other healthcare professionals

• Manage, support and train Foundation Trainee Pharmacists and pharmacy technicians

• Lead Quality Improvement projects and clinical audits

• Ensure safe and cost-effective prescribing, helping manage medicines post-hospital discharge.

About us

Integral to our Chronic Conditions Management process, this role will be a key member of the practice multi-disciplinary team (MDT). The post holder will use their specialist knowledge and prescribing skills to review medicines to manage long-term conditions. Practice pharmacists hold clinics for patients requiring face-to face or telephone Structured Medication Reviews (SMRs) and produce recommendations for the wider MDT on deprescribing, prescribing and monitoring, where clinically appropriate. This new post will support the training of pre-registration Foundation Trainee Pharmacists as part of the HEIW multi-sector rotational programme.

Alongside our experienced practice pharmacist, you will manage guide and support our practice pharmacy technicians, a cluster pharmacist, and prescription administrator. You will assist in the management of medicines on transfer of care, play an important part in developing and delivering systems for safer prescribing, as well as answering medicine-related enquiries or requests from GPs, other practice staff, and patients. The post-holder will take part in Quality Improvement projects and Clinical Audits, for example as part of the Medicines Management Incentive scheme and our GMS contractual obligations.

Job Responsibilities:

• Hold clinics for patients requiring face-to face or telephone Structured Medication Reviews (SMRs), prescribing, monitoring, and optimising medicines to manage long-term conditions.

• Contribute pharmaceutical advice for the development and implementation of new services/systems that have medicines/prescribing components (e.g. Chronic Disease Management, QI projects, LES).

• Provide management, support and training for prescription administrators, pharmacy technicians, and foundation trainee pharmacists.

• Contribute to public health campaigns, including flu vaccinations, and other adult immunisation programmes.

• Undertake changes to medicines (switches) designed to save on medicine costs in accordance with local formularies, or in response to disruptions in the medicines supply chain.

• Answer medicine-related enquiries or requests from GPs, other practice staff, and patients.

• Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

• Undertake audits of prescribing, feedback the results and implement changes in conjunction with the practice team.

• Monitor practice prescribing against the local health board formulary and make recommendations to GPs for medicines that should be prescribed by hospitals only or subject to shared care arrangements.

• Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

• Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients (e.g. those with medicine compliance aids or those in care homes).

• Undertake any other duties appropriate to the grade.

• Participate and support personal development reviews and objective setting with the Pharmacy hub.

• Maintain registration and revalidation with the General Pharmaceutical Council by regular updating, self-directed learning, and attendance at courses that would help in personal, and service, development.
