Depending on experience £40,400.21 - 40hours/week (negotiable, depending on experience)  PERMANENT  GOOD SALARY 

The practice nurse must provide and maintain a high standard in general practice nursing and chronic disease management for patients, which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals. They will provide assessment, treatment, screening, health education services and advice to patients, while adhering to practice protocols, policies, and guidelines. The post-holder must provide and maintain the highest possible standards of nursing care for patients as well as provide nursing assistance to the doctors and other members of the team. They will do this independently and with nurse and GP colleagues.

· All immunisations and vaccinations, including childhood immunisations.

· Child Health Surveillance.

· Contraception and sexual health advice and management.

· Travel Advice & Vaccines

· Routine Injections

· Hypertension and blood pressure monitoring.

· Health checks

· Cervical smear screening

· Asthma and COPD monitoring and management.

· Chaperone duties

· Chronic Disease Management

· Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals.

· Understand the aims of management and the pharmacological treatments for chronic disease.

· Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management.

· Development of recall procedures, appropriate literature and health promotion, and audit.

· Maximise QoF and Enhanced Services achievement and attend any training or updates necessary.

· Actively participate in audit activity and policy and protocol development for the evaluation and development of surgery’s care and service to patients.

· Produce accurate, contemporaneous, and complete records of patient consultations, consistent with the NMC and practice guidelines and protocols.

· Support other members the team at all times both clinical and non-clinical, including mentoring and teaching where desired.

· Take responsibility for their own work, as well as their learning and development at all times.

· Ensure the maintenance of equipment and stock levels relating to patient care.

· Comply with and follow the nurse revalidation programme.
