The role of Assistant Practitioner in the Children's Continence Service with Norfolk Community Health and Care NHS Trust involves providing essential care for children and young people with complex or acute health needs. The practitioner will manage their own caseload, including maintaining a waiting list and updating an online system, while planning and prioritizing care activities based on individual patient needs. The position requires the development, implementation, and evaluation of care plans in collaboration with families and carers, and the completion of delegated observations and assessments.

Additionally, the assistant practitioner will use effective communication skills to engage with children and their families, fostering understanding and participation in treatment. The role also involves working independently with children, contributing to the Education Health Care Plan (EHCP) process, managing an electronic diary for appointments, and adhering to safeguarding procedures.
