The role of Discharge Coordinator at Conquest Hospital, part of the East Sussex Healthcare NHS Trust, involves joining the Transfer of Care Hub to support coordinated and safe patient discharges in collaboration with health and social care teams. Candidates should have recent experience in health or social care, with hospital experience considered a plus, although full training will be provided. Strong customer care and communication skills are essential, along with a minimum education of GCSE or equivalent in literacy and numeracy.

Discharge Coordinators will work closely with multi-agency teams, participate in enhanced discharge meetings, and communicate with patients, their families, and carers to ensure timely discharge processes. The position requires flexibility for occasional cross-site cover and rotational weekend work. Personnel in this role will receive support in developing competencies and skills to enhance their effectiveness within the integrated discharge team.
