The role of Senior Mental Health Practitioner at East Lindsey PCN with Lincolnshire Partnership NHS Foundation Trust offers an exciting opportunity for professionals to contribute to the transformation of mental health services within primary care settings. As an autonomous practitioner working closely with specified GP practices, you will provide specialized mental health support and guidance to patients, their families, and wider NHS colleagues, ensuring a person-centered approach that promotes optimal physical, psychological, and social well-being. The position emphasises evidence-based, holistic assessments and aims to improve clinical outcomes by empowering individuals to manage their mental health effectively and remain in their communities, while minimizing unnecessary hospital admissions.

Key responsibilities include managing and prioritizing a diverse caseload, developing and implementing holistic care plans, and collaborating with multidisciplinary teams to enhance service delivery. You will also play a vital role in workforce development and support the learning needs of colleagues, contributing to service improvement and ensuring that best practices are adhered to.
