The role of Community Support Worker in the Learning Disability and Autism (LDA) Hospital at Home team, based in Oxford with Oxford Health NHS Trust, involves delivering intensive community support to young individuals and their families, aiming to prevent hospital admissions and facilitate smoother transitions from inpatient care.
