Job description
The post holder will be a Registered Health Care Professional either Nursing or Allied Health Care Professional, with experience and a special interest in Motor Neurone Disease (MND).
You will work as a part of a multidisciplinary team with the Rehabilitation Medicine Service within North and North East Lincolnshire, providing specialist support to patients from diagnosis to end of life care, including referral for families for bereavement support where required.
you will be expected to work closely with regional MND multidisciplinary teams (MDT) to enhance networks to ensure patients benefit from the most clinically appropriate and expeditious service available regionally or locally and that the needs of carers are also recognised and supported where possible.

Demonstrate clinical competence and expertise in neurological conditions, palliative care and end of life nursing, with a sound understanding of holistic needs of patients with MND and their families.
You will be responsible for the care and support of an identified patient caseload; attending the community MDT meeting and acute Trust MDT as required, sharing information and liaising with other MDT members including the Consultant Lead.
To ensure the patients' needs are met in a person centred way by the MDT and act as a resource for the wider MDT working alongside both medical, nursing and therapy team members, teaching and sharing knowledge/information.
You will be expected to develop excellent relationships with multiple key stakeholders including the rehabilitation service, hospice, palliative care teams, Neurology services within the acute Trusts, Speech and Language Therapy (SALT), home ventilation teams, dieticians etc to ensure care across community, hospice and hospital boundaries is seamless and patient centred.
The postholder will be involved with training and education of nursing and other staff groups as required.
