The Community Mental Health Practitioner/Community Psychiatric Nurse role at Surrey and Borders Partnership NHS Foundation Trust involves delivering a comprehensive nursing service to individuals with mental health challenges in the Guildford and Waverley area. The post holder will function as a Care Coordinator or Lead Professional, providing holistic care that emphasises patient autonomy and choice. Responsibilities include implementing community nursing services for older adults, prescribing appropriate care solutions, attending multi-disciplinary team meetings, ensuring safe medication administration, and maintaining high standards of professional practice based on best practices.
