The Newcastle upon Tyne Hospitals NHS Foundation Trust is looking for a motivated Band 6 Nurse Practitioner to join their intermediate care unit. This role focuses on providing high-quality rehabilitation care to patients, enabling them to maintain independence after hospitalisation. The Nurse Practitioner will work within a multidisciplinary team, conducting patient assessments and managing care to optimize health outcomes. Responsibilities include performing clinical evaluations, developing care plans, and facilitating communication among team members. The position supports professional development, including opportunities for prescribing studies (V300). The role involves a commitment to patient-centered care and effective resource management, with a maximum stay of six weeks for patients in the rehab unit. The Trust is open to flexible working arrangements. Interviews are scheduled for December 3, 2024.
