The Valentine Health Partnership is seeking a motivated and experienced Primary Care Network Advanced Clinical Practitioner (ACP) to join their team in Woolwich, employed through Greenwich Health. This role involves providing comprehensive patient care, including assessment, diagnosis, treatment, and evaluation for various acute and chronic conditions. Key responsibilities include:
Clinical Practice: Conducting thorough clinical assessments, developing treatment plans, prescribing medications, and managing both acute and long-term health issues. The ACP will assess complex needs, implement health promotion strategies, and support adherence to treatment.
Communication: Utilizing effective communication to inform patients, particularly in sensitive situations, and ensuring clarity in treatment plans.
Quality Service Delivery: Complying with professional standards and guidelines, maintaining accurate documentation, and engaging in self and peer evaluations to enhance care quality.
Leadership and Development: Mentoring junior staff, supporting team development, and contributing to clinical guidelines, while ensuring staff competency in line with service delivery goals.
Teamwork and Risk Management: Collaborating with the healthcare team to enhance patient care, identifying and managing risks, and ensuring compliance with health and safety regulations.
Information Management and Learning: Utilizing technology for patient management and audits, engaging in continuous professional development, and disseminating knowledge among team members.

This is an exciting opportunity to join the Valentine Health Partnership PCN. We are looking to recruit an enthusiastic, highly motivated and experienced Primary Care Network Advanced Clinical Practitioner.
The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients. The Post holder will work for Valentine Health Partnership, part of Valentine Primary Care Network.
An ACP will demonstrate critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
