The Valentine Health Partnership is seeking a motivated and experienced Primary Care Network Advanced Clinical Practitioner (ACP) to join their team in Woolwich, employed through Greenwich Health. This role involves providing comprehensive patient care, including assessment, diagnosis, treatment, and evaluation for various acute and chronic conditions. Key responsibilities include:

  1. Clinical Practice: Conducting thorough clinical assessments, developing treatment plans, prescribing medications, and managing both acute and long-term health issues. The ACP will assess complex needs, implement health promotion strategies, and support adherence to treatment.

  2. Communication: Utilizing effective communication to inform patients, particularly in sensitive situations, and ensuring clarity in treatment plans.

  3. Quality Service Delivery: Complying with professional standards and guidelines, maintaining accurate documentation, and engaging in self and peer evaluations to enhance care quality.

  4. Leadership and Development: Mentoring junior staff, supporting team development, and contributing to clinical guidelines, while ensuring staff competency in line with service delivery goals.

  5. Teamwork and Risk Management: Collaborating with the healthcare team to enhance patient care, identifying and managing risks, and ensuring compliance with health and safety regulations.

  6. Information Management and Learning: Utilizing technology for patient management and audits, engaging in continuous professional development, and disseminating knowledge among team members.
