As a Staff Nurse you will be responsible for providing care to people admitted to our ward for rehabilitation. We work closely as a multi-disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. . As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. Our nurses are required to facilitate the day to day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families, and carers to provided 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

Practice as a member of the Community Hospital Nursing team, to support and work in partnership with the wider multidisciplinary team.

Planning and Delivery of Individual Nursing Care under Direction of Senior Staff

Work within the NMC Code of Professional Conduct.

Assess, plan, and implement and evaluate patient focused nursing care.

Document care given and maintain contemporaneous records.

Maintain records in accordance with the Trusts policy and that of NMC Code of Professional Conduct.

Assist in implementation of health promotion strategies, as agreed by Ward Manager.

In consultation with senior qualified staff, provide and monitor patient centred care within Trust agreed standards.

In collaboration with other team members organise the reception and discharge of patients ensuring effective communication in transfer of information.

Carry out delegated nursing care.

Use and participate in research / audit in care delivery.

Adhere to and promote all clinical practice, health and safety and corporate Trust Policies and Guidelines.
