The Specialist Nurse position in the Hull & East Riding Memory Assessment Service at Humber Teaching NHS Foundation Trust focuses on providing diagnosis and treatment for dementia patients in a dynamic, multi-disciplinary team environment. The successful candidate should possess experience in dementia care, demonstrate a commitment to quality assessment and treatment, and have or be willing to obtain an Independent Prescriber qualification. Key responsibilities include triaging referrals, conducting assessments, delivering diagnostic feedback, and creating forward care plans. The role also involves clinical supervision of team members, managing local and national policy compliance, and fostering strong relationships with partner services.

Additionally, the Specialist Nurse must be confident in both independent and team settings, exhibit strong time management skills, and be prepared to work remotely. A critical aspect of the role is prioritizing patient safety and wellbeing, alongside promoting education for patients and their caregivers regarding dementia.
