Band 6: £37,338 to £44,962 a year per annum  PERMANENT  GOOD SALARY 

We have an exciting opportunity for an experienced Mental Health Practitioner to join our team to support the performance of 4Doncaster Primary Care Network (PCN). 4Doncaster is a forward thinking and innovative PCN looking to recruit a highly motivated, enthusiastic and versatile member of our team. The 4Doncaster PCN is 4 practices situated in central Doncaster:The Burns Medical PracticeThe Kingthorne Group PracticeThe Mount Group PracticeThe St Vincent Group Practice The practices came together in order to be able to respond in a flexible and responsive manner to changes taking place in the NHS. The establishment of the 4Doncaster network has enabled the recruitment of additional staff, including Clinical Pharmacists, Pharmacy Technicians, Physiotherapists, Social Prescribers, Mental Health Practitioners, Care Coordinatorsand Health & Wellbeing Coaches. This is a mental health role in primary care, it is an exciting model of working, situated at the heart of general practice while been employed by RDaSH. A GP integrated mental health service creates better joint working across primary and secondary care mental health systems. The post is direct patient facing and based at Rutland House and other sites across the network, consideration would be given to condensed working hours and part time hours- working days will be agreed collaboratively

Provide a role in supporting Primary Care Networks (PCN) and the band 7 Mental Health Practitioner in triaging mental health concerns and providing specialist advice.

Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system.

To act as a point of contact for GPs to gain advice and support regarding people's mental health.

Where appropriate liaise with the GP team and the PCN regarding complex situations.

To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.

Be effective in multi-professional team working and service transformation.

To provide a comprehensive Primary Mental Health Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery.
