Support the Dementia Outreach Service with the implementation of patient care plans, support and advice for care homes in the Broxtowe area.
Monitor and record individuals' mental health status and report any significant changes.
Assist individuals in developing and maintaining coping strategies to manage their symptoms.
Seeing patients who are permanent residents in Care Homes within the Broxtowe area.
Carrying out duty which involves making and receiving phone calls from care home staff offering support and guidance, reviewing their electronic records, and making entries.
Attending meetings face to face and on MS Teams.
Be a member of a multidisciplinary Team.
About usThis is an exciting opportunity, to be part of a specialist Outreach service for clients with dementia inresidential/nursing care providing specialist nursing/healthcare advice, treatment andteaching/education as appropriate under the guidance of professionally registered teammembers. To contribute to assessment and interventions carried out by the dementia outreach. to engage with the Enhanced Health in Care Homes Service (EHCH) andwith GPs in the local area.
Part of a multi-disciplinary Outreach service which works with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia. To increase the capacity, confidence, competence and willingness of those organisations to provide the right level of care to this vulnerable client group and by doing so to increase standards of care, improve quality of life, increase independence and choice. You will be supported through training and supervision.
As this role is community based covering the Broxtowe area, afull UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2.
Please note that this post does not meet the pay level required for a Skilled worker visa.
Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
