The Nottinghamshire Healthcare NHS Foundation Trust is seeking a Dementia Outreach Support Worker to assist with the implementation of patient care plans and provide support and advice to care homes in the Broxtowe area. Key responsibilities include monitoring and recording mental health statuses, helping individuals develop coping strategies, and communicating with care home staff. The role involves working in a multi-disciplinary team, attending meetings, and engaging with the Enhanced Health in Care Homes Service and local GPs.

Candidates must have a full UK driving license and vehicle for community-based work. The Trust emphasises staff development and offers support through training and supervision. Note that this position does not qualify for sponsorship under the Skilled Worker visa due to salary requirements.
