Join our compassionate team as a Community Heart Failure Nurse in Burton on Trent, where your skills can truly make a difference. We are looking for a dedicated and nurturing individual, preferably with a background in caring, who is passionate about empowering patients with heart failure. In this role, you will provide holistic, patient-centred care, working collaboratively with our Specialist Nurse-led team. Your goal will be to help patients achieve the best quality of life possible in their own communities, preventing hospital admissions and supporting their families along the way.

As part of Midlands Partnership NHS Foundation Trust, you will have the opportunity to make an impact from diagnosis through to end-of-life care. You’ll manage and monitor treatment plans, deliver intravenous therapies, and work closely with multidisciplinary teams to ensure that every patient receives the comprehensive care they deserve.
