The Health Improvement Practitioner role at Livewell Southwest in Plymouth is a part-time position (up to 18.75 hours per week) within the Falls Prevention Team, aimed at reducing falls and related injuries among older adults. The practitioner will utilize behavior change skills to encourage engagement in physical activity and falls prevention programs while conducting multifactorial assessments for at-risk individuals. Key responsibilities include leading the Falls Management Exercise (FaME) program upon completing the Postural Stability Instructor course, providing both group and one-on-one interventions, and collaborating with a multi-disciplinary team, including physiotherapists.

Additionally, the role involves outreach efforts to engage hard-to-reach populations in health improvement initiatives, participating in community events, and contributing to the development of training programs for other professionals. The position is fixed-term for two years, with a focus on continuous learning and the application of evidence-based interventions to enhance the wellbeing of the community.
