Cardiac Adolescent and Transition Clinical Nurse Specialist Band 6 1.0 WTEAre you looking for a new, dynamic challenge developing and improving the transitional care of young people with congenital and acquired heart disease? An exciting opportunity has arisen for an experienced, motivated, and enthusiastic nurse to join the cardiac adolescent and transition serviceWe are looking for an experienced cardiac nurse with vision and enthusiasm for delivering holistic young person-centred care. The role includes service development of transitional care for youngpeople with congenital and acquired heart disease, leading a small, motivated team, whilstmanaging a caseload of young people with congenital heart disease (CHD) and acquired heartdisease. You must have excellent leadership and management skills, experience in service development and improvement and aspire to develop innovative teaching strategies, whilst alsobeing able to quickly develop a positive rapport with young people. The role will involve developing strategies to empower young people to manage their on-going health needs as they prepare for adulthood, whilst implementing pathways to reduce patients becoming lost-to-follow-up.We will support your on-going development and educational pathway to achieve these goals. This isa full-time position.

  • To work as an active member of the team with the ATCNS to provide a high quality transition service for inpatients and outpatients across the network and out in to the community
  • To work within the Trust's Generic Policies and Procedures and the parameters set down in agreed local clinical protocols and guidelines and national guidelines such as those published by NICE
  • To use the latest evidence based practice and research and to develop an evidence base with support of ATCNS in areas where this is lacking
  • To develop the confidence to proactively engage other members of the MDT and be a spokesman for your speciality
  • To act as a resource, providing clinical leadership and facilitating professional development amongst the multidisciplinary team with supervision from the ATCNS
  • To work closely with the Clinical Leaders and Practice Educators to provide dynamic and strong education to the clinical unit and out in to the network to meet the current care delivery needs
  • To act as a role model to members of the multiprofessional team demonstrating excellence in transition and adolescent care, providing expert knowledge and skills whilst empowering other members of the health care team
