North Staffordshire Combined Healthcare Trust is looking for a dedicated Outreach Worker to join our Reconnect Service, where you'll play a vital role in supporting people navigating the complexities of the criminal justice system. This part-time position offers 22.5 hours a week, allowing you the flexibility to balance your career and personal commitments.
In this impactful role, you'll provide holistic support to individuals released from prison, guiding them through their health and social care needs while helping to reduce the risk of reoffending. We’re seeking someone with excellent communication skills who is non-judgemental and empathetic—qualities that will empower those you work with to achieve their goals. If you’re ready to collaborate with various agencies and make a meaningful change in the community, we’d love to hear from you! Please note that previous applicants need not apply.
