The Community Psychiatric Nurse role at Surrey and Borders Partnership NHS Foundation Trust in Chertsey focuses on providing high-quality mental health care primarily for older individuals, particularly those over 65 with severe mental health issues, cognitive decline, or dementia. The position involves acting as a key worker and care coordinator within the Care Programme Approach, ensuring comprehensive assessments, treatment, and support are delivered effectively. Nurses will work closely with families, social services, and voluntary organisations like the Alzheimer's Society to enhance the care experience for clients and their caregivers.
