To provide recovery-based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.
This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence-based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
