Greenwich Health deliver NHS Health Checks, smoking cessation support, Long-Acting Reversible Contraception (LARC) services and sexual health walk in clinics. We are looking to recruit a flexible and motivated individual who can work both independently and as part of a team in a fast paced patient centred role.
The post holder will be required to conduct NHS Health Checks, face to face and telephone smoking cessation consultations and provide HCA support for our Long-Acting Reversible Contraception clinics. The post holder will be required to travel to various locations across the Borough of Greenwich.
The post holder will also be required to support our busy patient call centre based at Ferryview Health Centre, supporting patients through the service from managing the referral list, triaging, and booking patients into clinics.
Working Pattern: 40 hours per week (working hours between 8:00am and 8:00pm, 5 days a week Monday to Sunday on a three week rota, 1 weekend day 8am to 4pm every third week)

Deliver the NHS Health Check sessions across Greenwich within the Live Well Centres.

Providing telephone or face to face support to patients wishing to quit smoking.
Advise patients on nicotine replacement therapy options.
Provide on-going support to patients throughout their 12 week quit journey with a patient centre approach.
Long Acting Reversible Contraception Clinic
Act as a LARC assistant to our LARC clinical staff, ensuring the patients feel comfortable and the clinician has appropriate, timely support.

Job Summary
The post holder will also be required to support our busy patient call centre based at Ferryview Health Centre, supporting patients through the service from managing the referral list, triaging, and booking patients into clinics.
The NHS Health Check aims to help delay or prevent heart disease, stroke, diabetes, kidney disease and dementia. Individuals between the ages of 40 - 74, who have not already been diagnosed with one of these conditions or do not have certain risk factors, will be invited every 5 years to have a check to assess their risk and will be given advice to reduce or manage that risk. The NHS Health Check involves measuring blood pressure and pulse, taking a near-patient blood sample to measure cholesterol, measuring height, weight and BMI and asking questions and providing lifestyle advice.
Our Smoking Cessation service supports patients for up to 12 weeks with either face to face or over the telephone to quit smoking. The post holder will have a patient case load to support through their smoking journey.
The post holder will support clinicians in our LARC clinics preparing and clearing away clinical equipment for coil and implant procedures, supporting patients during the procedure. The post holder will provide administration support the sexual health service.

Make timely referrals, following a health check, to general practice as required.
Prepare, organise and collate own paperwork, equipment and resources ready for delivery.

Process smoking referrals from practices, contacting patients within 72 hours.
Providing telephone or face to face support to patients wishing to quit smoking.
Advise patients on nicotine replacement therapy options.
Provide on-going support to patients throughout their 12 week quit journey with a patient centre approach.
Prepare, organise and collate own paperwork, equipment and resources.
Long Acting Reversible Contraception Clinic
Support LARC clinics following protocols and clinical guidelines.
Process the referrals from practices into the LARC clinics, triage and provide accurate information to ensure the patient attends the appropriate appointment and is aware of the arrangements.
Act as a LARC assistant to our LARC clinical staff, ensuring the patients feel comfortable and the clinician has appropriate, timely support.
Prepare, organise and collate own paperwork, equipment and resources ready for delivery.
Training
Complete NCSCT e-learning Stop Smoking Practitioner Training
Complete both internal and external NHS Health Check training.
Complete practical training alongside colleagues and clinicians for LARC clinics.
Complete all mandatory company training, including annual update / refresher training.
Attend monthly team meetings.
Take an active part in developing own knowledge and skills, particularly the key skills and competencies required to deliver NHS Health Checks, smoking cessation and LARC support.
Keep abreast of changes within the health industry via the reading of literature, attending seminars and continuing professional development
Health & Safety
Monitor and maintain health, safety and security of self and others.
Respond to relevant clinical /other issues or incidences to mitigate risk and maintain best practice.
Follow information governance and safeguarding procedures.
