A high profile, accessible and responsive leader with managerial and clinical responsibilityfor the delivery of Advanced Clinical Practice across Cardiology and the emergency and acute footprint. Able to make informed decisions within their own speciality and scope ofprofessional practice, involving patients and public to ensure agreed standards, targetsand objectives are met.
The post holder is an experienced professional qualified to Masters Level, who will act asa clinical leader within the Cardiology Service to provide a service workingalongside the Cardiology Consultants and nursing team. They will autonomously clinically assess and manage patients referred to the Cardiology service in conjunction with the consultant to support the faster diagnosis including referrals from the emergency department, the acute medical footprint and face to face/virtual clinics.
The post holder will be instrumental in providing relevant education for patients and lifestyle advice for their condition. The post holder will be required to ensure evidence-based care is delivered to patients and will be involved in the audit of practice, including leading on the Trusts contribution to service improvement to achieve the National standards in collaboration with consultant colleagues.
Clinical
Co-ordinate the care for Cardiology patients throughout their pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
Lead and deliver clinical care, across a variety of settings, according to national and local guidelines ensuring delivery of evidence-based care and working towards the Trust vision of providing a World Class Service.
Undertake an initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision-making skills, deciding when necessary to refer to senior clinical colleagues.
