DEMENTIA MENTAL HEALTH NHS SOCIAL WORK
The Enhanced Care Practitioner at East Lancashire Hospitals NHS Trust is responsible for supporting the Lead Nurse in implementing the Enhanced Care Standard Operating Procedure across various sites. This role involves managing and supervising Enhanced Care Support Workers, providing clinical leadership, and ensuring effective care delivery for patients at risk due to cognitive disturbances, such as those with dementia, learning disabilities, or mental health issues. The practitioner will develop and monitor patient-centered care plans, utilize appropriate intervention strategies, and support ward teams in improving their knowledge and skills regarding enhanced care.

Additionally, this role entails line management responsibilities for Enhanced Care Support Workers, including clinical supervision and performance appraisals. The practitioner will collaborate with the Lead Nurse to develop policies, conduct incident investigations, and oversee audit activities, all aimed at safeguarding vulnerable patients and enhancing their hospital experience in a supportive, therapeutic environment. The ideal candidate will possess extensive knowledge of caring for vulnerable populations and demonstrate a commitment to promoting patient safety and well-being while engaging collaboratively with the healthcare team.
