The Mid Yorkshire Teaching NHS Trust is seeking an Advanced Clinical Practitioner (ACP) to join their team in Wakefield, aiming to innovate community healthcare delivery. The successful candidate will be a qualified practitioner with a strong background in managing undifferentiated and undiagnosed conditions, demonstrating high levels of clinical autonomy, complex decision-making, and leadership skills. They will work collaboratively within a multidisciplinary team to improve patient care outcomes, manage urgent referrals, and provide both clinical care and teaching to nursing and medical staff.

Key responsibilities include conducting comprehensive patient assessments, making clinical diagnoses, and determining appropriate referrals, alongside the ability to discharge patients post-treatment.
