Job overview
The Care and Support Worker will work within a day service setting supporting people who have a wide range of learning, physical and health needs and also includes people who need support when they become distressed. You will participate in and access activities in the centre and the community and work as part of a multi-disciplinary team working with people who attend Hollacombe, their parents, carers and other professionals'. You will provide support in accordance with individuals care plans and carry out tasks allocated by the senior team.
You will act as an escort on a minibus with up to 8 other passengers with a tail lift and have responsibility alongside the driver for the safe transportation of people to and from the day service.
The service operates within the policies and procedures of Torbay and South Devon NHS Foundation Trust and relevant national legislation and guidance e.g. Valuing People Now, Mental Capacity Act, and Safeguarding Adults etc.

To work sensitively with people who have communication difficulties and develop relationships with them using appropriate communication tools e.g., signs, symbols, body language, structured language- as defined by their care plans. Endeavour to use Person Centred Approaches with people as a way of making sure that we do a good job of listening to and responding to what an individual really wants and needs. Communicate professionally and effectively with colleagues, other professionals, parents and carers. Be responsible for accessing emails and other relevant on line systems.
