The Care Navigator role with Cheshire and Wirral Partnership NHS Foundation Trust in Birkenhead involves providing support to young people and their families awaiting treatment within the Children & Young People's Mental Health Service. The position focuses on community engagement, requiring access to transport to meet with families, attend multidisciplinary meetings, and assist young people in accessing various community services. The Care Navigator will also be responsible for developing care plans and raising concerns regarding any deterioration in the mental health of the young individuals they support.
