The role of Band 4 Therapy Assistant in the Care Home Inreach Team at St George's University Hospitals NHS Foundation Trust involves collaborating with a multidisciplinary therapy team to enhance the health and wellbeing of residents in Wandsworth's nursing and residential homes. The successful candidate will work closely with physiotherapists, occupational therapists, and other health professionals to deliver tailored therapy sessions, training, and support to both residents and care home staff. Key responsibilities include developing and executing therapy activity programs, providing administrative assistance, maintaining care home relationships, and contributing to service improvement while promoting principles aimed at physical fitness and cognitive stimulation to reduce hospital admissions and prevent deterioration of residents’ health.
