Plas Menai Health Centre is seeking a part-time Care Navigator to join their team in Llanfairfechan. The role requires 20 to 30 hours of work per week and emphasizes excellent customer service and support for patients in a fast-paced environment. Key responsibilities include:

Processing appointment requests and repeat prescriptions
Handling patient queries and liaising with pharmacies and medical staff
Managing phone calls, emails, and administrative tasks
Ensuring effective communication and teamwork within the practice.
