The role of Primary Care Nurse at HMP & YOI Feltham, with Central and North West London NHS Foundation Trust, involves providing trauma-informed primary care, mental health, and substance misuse services to young people and young adults in custody. The position requires a Registered Adult or Children’s Nurse to join an Integrated Healthcare Service, where they will carry out clinical assessments, plan and implement patient care, and serve as a primary point of contact for healthcare needs. The nurse will work closely with operational colleagues to promote health and wellbeing while adhering to 'Child First' principles, ensuring that care is tailored to the specific needs of the young population.

The successful candidate will engage in various nursing duties, including administering medication, conducting health promotion clinics, and supporting patients requiring mental health or substance misuse interventions. Furthermore, the nurse will maintain accurate records, assist with wound care, and facilitate a seamless discharge process for patients.
