CASELOAD DEMENTIA MENTAL HEALTH MIDWIFERY MULTI-DISCIPLINARY NHS PENSION
The Norfolk & Suffolk Foundation NHS Trust is seeking a Community Mental Health Nurse to join the Ipswich Older People’s Community Mental Health Team. In this role, the nurse will be responsible for planning and delivering patient care with minimal supervision, managing a caseload of individuals with mental health needs, conducting mental health clustering, and ensuring continuity of care across multi-disciplinary and multi-agency teams. Key responsibilities also include conducting risk assessments, evaluating patient needs, and facilitating patient-centered reviews and case conferences.

Additionally, the Community Mental Health Nurse will mentor junior staff, maintain accurate clinical documentation, and comply with relevant policies and guidelines.
