As a Practice Nurse, you will play a vital role in delivering high-quality care to our patients. With a focus on respiratory health, you will provide expert support to patients with conditions such as asthma, COPD, and other respiratory disorders. You will work closely with GPs, other nurses, and the wider multidisciplinary team to ensure the best possible outcomes for our patients.
This part-time post will be up to 25 hours per week over 3 days but we have some flexibility with this for the right candidate.
Conduct chronic disease management, particularly in asthma and COPD clinics.
Perform diagnostic and monitoring tests, such as spirometry (if trained).
Educate patients on managing their respiratory conditions and promote self-care.
Administer vaccinations, including flu and pneumococcal immunisations.
Carry out general practice nursing duties, including wound care, cervical cytology, and health checks.
Work collaboratively with the team to improve respiratory care pathways and patient outcomes.
The post holder will use their knowledge of best and evidence based practice and the GMS contract (including the Quality Outcomes Framework) to strive to deliver, promote and improve standards of care.
Ideally will focus on respiratory health providing expert support to patients with conditions such as asthma, COPD, and other respiratory disorders.
They will be involved in identifying practice population needs and in being proactive in making suggestions for service improvement.
The post holder is expected to undertake any training and development necessary to keep their knowledge and practice up to date and current.
The post holder will work directly within the Practice Nursing team.
