The Central London Community Health Trust is seeking a Rehabilitation Assistant to join its Tri-Borough Falls Prevention Service, which serves residents at risk of falls in Westminster, Kensington & Chelsea, and Hammersmith & Fulham. The role involves working collaboratively with Physiotherapists and Occupational Therapists to deliver strength and balance classes, while also conducting independent assessments in clinic settings to evaluate exercise, memory, balance, and blood pressure. Additionally, the Rehabilitation Assistant will provide home visits to enhance clients' mobility and safety, following the guidance of therapists.

The successful candidate will receive training on various assessment tools and will be responsible for delivering individualized care aimed at promoting independence for a delegated caseload. Working in diverse environments such as community centers and patient homes, the Rehabilitation Assistant will also contribute to achieving performance and quality targets within the Falls Prevention Team.
