The Advanced Practitioner Integrated Urgent Care plays a critical role in providing clinical leadership, supervision, and governance to Specialist Paramedics and Nurses engaged in remote urgent and emergency care assessments. This position supports clinical workforce development, ensuring the safe and effective delivery of care in alignment with the Trust's clinical strategy.

Using advanced skills and knowledge, you will practice through comprehensive patient assessments, diagnostics, and interventions, particularly for complex cases.

As the lead urgent care clinician supporting clinical validation, you will ensure patient safety and care quality through clinical leadership, development, and supervision. Key responsibilities include analysing performance indicators, formulating clinical development and risk mitigation plans, and supporting continuous clinician proficiency through education and development.

Aligned with national frameworks in advanced clinical practice, this role focuses on the four pillars of advanced practice: expert clinical practice, clinical leadership, research and audit, and education. To maintain competence, you will rotate through the primary and urgent care system and the Emergency Operations Centre (EOC).

- Provide clinical leadership to the Trust's workforce for integrated urgent care.

- Ensure ambulance clinical quality indicators and key clinical performance metrics are achieved.

- Offer subject matter expertise in clinical investigations and reviews, making balanced judgments on care standards in line with the Just Culture guide.

- Deliver the advancing practice program within the clinical validation service and integrated urgent care.

- Conduct clinical case and practice reviews within the clinical validation service and integrated urgent care.

- Supervise specialist paramedics/nurses and other clinical staff.

- Implement the clinical strategy within the clinical validation service and integrated urgent care, ensuring audit results, investigation findings, research outputs, and clinical education plans enhance practice.

- Provide clinical advice and support to operational management, quality & safety, research, and clinical pathways teams.

- Practice according to the four pillars of advanced practice through clinical work, research, education, and leadership to advance professional development.

Patient Care

The post holder will manage patient care by:

- Conducting advanced clinical assessments of patients with complex health needs, ensuring care plans meet these needs.

- Employing structured clinical referral and handover processes.

- Using clinical judgment to assess, diagnose, treat, refer, and/or discharge patients with undifferentiated injuries and illnesses.

- Adhering to Trust, JRCALC, and national guidelines while making decisions within their professional scope of practice.

- Utilising advanced clinical skills and knowledge defined in the Advanced Practitioner Urgent Care Scope of Practice.

- Administering or prescribing medication in line with Trust policies and guidelines.

- Navigating acute and urgent care pathways effectively, including arranging direct admissions when necessary.

- Referring patients to appropriate care settings and ensuring proper handover.

- Maintaining accurate records and understanding information governance responsibilities.

- Delivering evidence-based care and maintaining professional standards (e.g., ILS, JRCALC, NICE).

- Treating patients, relatives, and the public with dignity and respect, even in difficult situations.

Research and Audit Responsibilities

- Promote the service by collecting, analysing, and reporting evidence of its impact on patient outcomes.

- Lead research, clinical audit, service evaluation, and improvement initiatives.

- Contribute to developing guidelines and care pathways based on evidence.

- Participate in serious incident reviews and embed learning into practice.

- Serve as an expert resource within the organisation.

Leadership, Training, and Supervision Responsibilities

- Engage in self-appraisal and lead others development through supervision, mentorship, and reflective practice.

- Act as an expert resource and role model, demonstrating high standards of professional and clinical practice.

- Supervise and support Specialist Paramedics/Nurses, ensuring their practice development.

- Mentor clinicians and support their CPD and revalidation training.

- Develop systems for feedback to improve clinical care and professional practice.

- Support training and development activities across the organisation.

- Drive personal CPD, including team development reviews.

- Collaborate with system partners to develop service improvements.

- Maintain a professional portfolio to evidence scope of practice and CPD.

- Undertake statutory and mandatory training as required.

Communication and Relationship Responsibilities

- Build working relationships with colleagues, peers, and managers across the Trust.

- Provide trusted supervision and feedback to paramedics, nurses, and other clinicians.

- Develop relationships with external organisations to lead system working initiatives and represent the Trust in clinically relevant forums.

Analytical and Judgmental Responsibilities

- Conduct advanced clinical assessments of patients with complex needs and plan appropriate care.

- Exercise clinical judgment to assess, diagnose, treat, refer, and/or discharge patients within Trust guidelines.

- Act as a clinical leader with expertise in remote assessment and management of various conditions.

- Analyse clinical data related to patient care and research activities.

- Make reasoned judgments based on clinical evidence and practice.

Planning and Organisational Responsibilities

- Self-manage and prioritise workload, balancing clinical supervision with governance and development activities.

- Work in the Emergency Operations Centre, providing remote clinical advice and decision support.

- Represent the organisation in public and professional settings, developing partnerships to enhance service and practice.

Policy and Service Development Responsibilities

- Promote a positive safety culture by contributing to policy and guideline development.

- Lead the development of evidence-based SOPs, clinical guidelines, and care pathways.

- Lead initiatives related to service development that improve patient care.

Financial, Physical, and Information Resource Responsibilities

- Manage medicines in line with Trust policies and ensure the security of Trust vehicles and premises.

- Use Trust equipment, facilities, and premises appropriately, ensuring safety and security.

- Comply with Health and Safety regulations to maintain a safe working environment.
