This is a relatively new role working for the Sherborne Area Primary Care Network, consisting of three local practices - The Grove Medical Centre, The Apples Medical Centre and Yetminster Health Centre. You will be helping to improve patient access to care and contributing to the development and improvement of processes. Duties will vary depending on current NHS initiatives. The role requires working across all three practices, plus meetings and off-site training so a full UK driving licence with own transport is a necessity. The hours required are between 22.5 to 30, with days and daily hours to be agreed. You will be joining our ever growing PCN and will work closely with the other team members such as the Social Prescribers and Pharmacy Team.
The main duties will include:
- Administrative support to the PCN Operational Manager and Pharmacy Team
- Improve patient access to digital resources, including the NHS App and practice websites.
- Complete finance-based tasks as required using Excel and Xero, with awareness of budgetary restrictions
- Support the implementation and progression of current and evolving PCN projects and contracts, compiling reports and using data analysis to provide feedback
- Support practices with data coding on clinical systems, ensuring coding is compliant and consistent.
- Be aware of and communicate both local and national health care campaign
- Provide support to colleagues, including resources on digital tools and systems
About us
The Digital Care Coordinator will support patients and colleagues in the use of digital healthcare technology, whilst providing administrative support to the PCN Team. You will possess excellent organisational and communication skills, whilst demonstrating an aptitude for digital resources. Competent using Word, Excel, and PowerPoint.
The role will be 22.5 to 30 hours per week, days and hours to be agreed. Full UK driving license with own transport is required as you will work across three practices and attend meetings and training off site.
Responsibilities:
- Provide administration support to the PCN Operational Manager, and the PCN Pharmacy Team
- Work with the PCN Operations Manager and the managers and GPs at each member practice to support the effective delivery of the PCN requirements
- Support the implementation and progression of current and evolving PCN projects and contracts, compiling reports and using data analysis to provide feedback
- Develop and maintain an understanding of the systems used by the NHS and healthcare, aligning them with our PCN digital infrastructure
- Provide support to colleagues, including resources on digital tools and systems
- Support integrated working, building relationships and facilitate collaboration between practices to support the delivery of care to our patients
- Improve our patients access to digital resources, including the NHS App and practice Websites
- Support practices with data coding on clinical systems, ensuring coding is compliant and consistent
- Be aware of and communicate both local and national health care campaigns, ordering resources and promoting via practice websites and other media
- Awareness of budgetary restrictions, with finance-based tasks as required using Excel and Xero
Organisational:
- Be the main contact for information technology resources within the PCN
- Undertake administrative tasks to help ensure the smooth running of the PCN, supported by the Operations Manager
- Support the strategic and operational goals for the PCN, assisting with the drawing up and implementation of action or project plans
- Working with the practices, assisting with administration regarding inviting patients for specific PCN services or contracts
- Support the PCN and practices with data-related reporting and analysis as required
- Update and communicate relevant information to PCN member practices
