We have an exciting opportunity for a Respiratory Specialist Nurse to join our Primary Care team for the London Region. This is a brand new role and you will have the autonomy to manage your own caseload in a unique setting.
The London region currently covers six sites including, HMP Belmarsh, HMP Thameside, Heathrow IRC, HMP Pentonville, HMP Wormwood Scrubs and HMP Brixton.
In this role you will be working across all sites with the possibility to work from home 1 day a week.

Monday to Friday - 9.00 to 17.00
Salary
This position is full time, youll receive an annual salary up to £58,025 per annum FTE - depending on experience.

As a Respiratory Specialist Nurse, your responsibilities will vary. You will
- To act as a specialist practitioner in the assessment and treatment of patients with respiratory disease (COPD, Interstitial Lung Disease and Bronchiectasis) and other healthcare professionals whilst working collaboratively under the NMOC framework. This will be to provide the best care and support to patients and staff through an integrated approach.
- This will include a specialist respiratory holistic assessment, to assess, plan, implement and evaluate all aspects of patient care including, QoF, referrals to specialist pathways and developing individualised treatment/care plans.
- Conduct Annual Long Term Condition (Respiratory) reviews in line with Health in Justice 7A indicators and service specification.
- Offer health promotion advice and support to patients with respiratory disease including smoking cessation where appropriate.

About us
As a Respiratory Nurse Specialist working within Health in Justice (HiJ) you will focus upon the autonomous assessment, management, review and supportive care of patients living with a respiratory diagnosis including COPD, Interstitial Lung Disease, Bronchiectasis and Asthma. This will include admission avoidance and supporting patients and staff post discharge from hospital as well. You will work across the London region within our HiJ establishments and support developing robust respiratory pathways and quality improvement projects.
Within our Practice Plus Group (PPG) HiJ establishments we are moving away from a tradition medical model, where teams work in silo, to an integrated care pathway approach (New Models of Care (NMOC)), which follows the patient journey. As the Respiratory Nurse Specialist, you will support the care planning in each pathway, all patients regardless of where they are in their journey to ensure shared care and decision making.
Applicants will demonstrate extensive respiratory knowledge with appropriate post registration qualifications and excellent communication skills. An understanding of the current drivers in respiratory care nationally to transform services is an essential requirement. Quality improvement and patient safety is a core ethos of the service.
Main duties of the job
To act as a specialist practitioner in the assessment and treatment of patients with respiratory disease (COPD, Interstitial Lung Disease and Bronchiectasis) and other healthcare professionals whilst working collaboratively under the NMOC framework. This will be to provide the best care and support to patients and staff through an integrated approach.
This will include a specialist respiratory holistic assessment, to assess, plan, implement and evaluate all aspects of patient care including, QoF, referrals to specialist pathways and developing individualised treatment/care plans.
Conduct Annual Long Term Condition (Respiratory) reviews in line with HiJ 7A indicators and service specification.
Promote and encourage self-management for patients living with respiratory disease including provision of personalised self-management plans for patients where appropriate and rescue medication packs.
Educate and empower patients and carers to identify early signs of change in their condition and provide them with the necessary knowledge and skills to gain independence and make informed choices to safely manage their condition. This includes providing individualised self-management plans and use of Healthcare Applications.
Offer health promotion advice and support to patients with respiratory disease including smoking cessation where appropriate.
Evaluating service provision and effectiveness and developing integration as directed by the service specification.
To support the delivery of respiratory education and training across the HiJ London Region.
HiJ aims to deliver a service which is equivalent to the community, however, the role will include working in collaboration and where necessary review patients who are under secondary care or specialist care services to bridge the gap and achieve continuity of care.
