The Central London Community Health Trust is seeking a Specialist Physiotherapist to join the Brent Community Rehab's Discharge to Assess Team. The role involves collaborating with Brent Adult Social Care to provide high-quality reablement services to community residents. Candidates should have a relevant professional qualification and experience in community rehabilitation.
