The Therapeutic intervention service delivers both group work and individualised work to people of all ages newly diagnosed with dementia and living in the community.
Integral to the service is the strong commitment to support families and carers given by the team family peer support workers.
The role of the assistant practitioner is patient facing and delivering group based educational programmes over an 8 week period at a variety of sites across the county.
The role is primarily concerned with delivering group based educational information to people of all ages diagnosed with dementia across the county. This includes maintaining accurate records and supporting the wider team to safely manage the waiting lists.
This role requires excellent communication skills both digital and verbal and a confidant manner when speaking to the public.
You will be required to have some knowledge and experience of working with people diagnosed with dementia or other mental illness and be able to offer support and care to them whilst they are attending the group.
You need to be able to work effectively within a team whilst being supported by registered staff.

You will be confidant speaking to members of the public and be able to modify your communication style as required by the audience and situation.
You will have an understanding of dementia and be able to provide care and support as required.
You will act professionally at all times and demonstrate a positive image of the service.
You will maintain accurate records and know your limitations and when to escalate risk and safeguarding concerns to the registered team members.
You will support the team to engage with families on the waiting list for the service.
You will be involved in all aspects of the service and team working as required.
You will be supported by registered staff and full training will be given including monthly clinical supervision.
Please note that this post does not meet the pay level required for a Skilled worker visa.
Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
