The Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a qualified and dedicated Care Coordinator for the Southampton West Older Persons Community Mental Health Team, located at the Western Community Hospital in Millbrook. This Band 6 position involves conducting mental health assessments, offering support, and coordinating care for elderly clients facing severe mental health challenges. Ideal candidates are preferred to have experience working with the elderly, particularly those with dementia, and should possess strong communication skills, a commitment to person-centered care, and the ability to work collaboratively within a team.

As part of the role, the Care Coordinator will manage a complex caseload, develop care and risk management plans in collaboration with service users and their families, and supervise lower-band practitioners when appropriate.
