The "Specialist Physiotherapist - Community Rehabilitation Team" position at Calderdale and Huddersfield NHS Foundation Trust involves providing high-quality assessment, treatment, and advice to adults with diverse rehabilitation needs in a community setting. The role entails managing a clinical workload autonomously, focusing on various patient groups with conditions such as strokes, neurological disorders, orthopaedics, and complex multi-morbidities. The specialist physiotherapist will develop and implement individualized treatment plans, set SMART goals, and collaborate closely with a multidisciplinary team (MDT) to ensure efficient patient outcomes and transitions in care.

Additionally, the role requires participation in governance initiatives, clinical supervision of staff and students, and contribution to the continuous development of clinical protocols and best practices based on current evidence. The specialist physiotherapist will also be responsible for maintaining safety in the use of equipment and for supporting the MDT in achieving their objectives, alongside the ongoing professional development of themselves and their peers.
