The Community Occupational Therapist position with Norfolk Community Health and Care NHS Trust in King's Lynn is a Band 6 role within the Intermediate Care team, which focuses on providing high-quality, evidence-based rehabilitation care for patients both in rehabilitation beds and in their homes. The therapist will be responsible for conducting comprehensive assessments, formulating individualized treatment plans, and facilitating early hospital discharges while preventing unnecessary admissions. Effective communication, problem-solving abilities, and experience in discharge planning and rehabilitation are key requirements for this role.

The successful candidate will collaborate with a multidisciplinary team and external agencies to ensure seamless care for patients with long-term conditions, the elderly, and those requiring palliative care. Responsibilities include assessing functional abilities, providing support and education to enhance patient independence, utilizing assistive technology, and managing patient records.
