The Marlow Adult Community Health Care Team (ACHT) at Buckinghamshire Healthcare NHS Trust is seeking a full-time District Nurse/Community Case Manager Nurse to work 37.5 hours per week. The role focuses on providing integrated, patient-centered care in the community, emphasizing caseload management, prevention of hospital admissions, reablement, and supporting end-of-life care. Candidates will be involved in facilitating timely hospital discharges and managing a caseload of patients, particularly those with long-term conditions and high healthcare needs.

The successful candidate will be responsible for assessing patients' physical and psychosocial needs, delivering evidence-based therapeutic treatments, and improving health outcomes through specialized nursing skills. Additionally, the role involves proactive case finding for high-intensity users of community healthcare and developing case management programs for referred patients. The ACHT aims to deliver coordinated care in partnership with various key stakeholders to enhance the overall quality of health services provided to the local population.
