The Street Healthcare Outreach Nurse position in Croydon with Change Grow Live is focused on providing essential healthcare interventions to individuals experiencing homelessness and substance use issues. This role is part of a new project initiated by government agencies aimed at evaluating effective strategies to reduce rough sleeping and improve health outcomes for the homeless population. The nurse will work within outreach teams, manage a caseload, conduct comprehensive health assessments, and deliver various clinical treatments while advocating for access to mainstream health services. The position involves unsociable hours, including early morning and late-night shifts, as well as working in challenging weather conditions.

The nurse will establish therapeutic relationships based on trauma-informed care, make high-level clinical decisions, and initiate complex interventions. Responsibilities include conducting mental capacity assessments, identifying and addressing health risks, delivering vaccinations, and promoting harm reduction initiatives. The role also requires collaboration with various health and social care services to ensure coordinated care and address safeguarding concerns. Documentation of clinical activities and patient feedback will be essential, alongside participating in ongoing training and maintaining communication with the lead nurse and other partners.
